Восстановление функции удержания мочи у пациентов после нервосберегающей радикальной простатэктомии (реферат, курсовая, диплом, контрольная)

1. У пациентов, перенесших НС-РПЭ, восстановление функции удержания мочи происходит быстрее, чем у пациентов, у которых не была использована нервосберегающая техника. Через 12 месяцев наблюдения в обеих группах отмечено НМ только легкой степени. В группе пациентов, перенесших НС-РПЭ, частота НМ ниже (2,74%), чем в группе, где РПЭ выполнялась без использования нервосберегающей техники (9,57%).
2. Выполнение РПЭ с использованием нервосберегающей техники к 3 месяцу наблюдения уменьшает частоту и степень выраженности исходной ирритативной симптоматики на 45%, в группе пациентов с восстановленной ЭФ — на 59,1%, в группе сравнения — на 39,14%.
3. В группе пациентов с восстановленной ЭФ к третьему месяцу наблюдения подавляющее большинство пациентов полностью удерживало мочу (93,44%). В группе после НС-РПЭ в целом этот показатель составил 87,67%, а в группе сравнения, где СНП не сохранялись, удержание мочи отмечено у 57,12% пациентов.
4. Восстановление ЭФ после НС-РПЭ является значимым фактором прогноза восстановления функции удержания мочи в послеоперационном периоде (г = -0,6, р< 0,05).
5. При персистенции ирритативной симптоматикой после РПЭ назначение препаратов из группы М-холиноблокаторов уменьшает степень тяжести СНМП.
ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ
1. В случае планируемой РПЭ с применением нервосберегающей техники необходимо тщательно объективно оценивать онкологический статус, исходную функцию удержания мочи и эректильную функцию пациента.
2. При использовании нервосберегающей техники РПЭ рационально сохранение обоих СНП. Необходимо использовать увеличительные линзы и по возможности не применять электрохирургические методы на этапе выделения и сохранения СНП. В зоне их проекции целесообразно высокое вскрытие эндопельвикальной фасции ближе к передней поверхности предстательной железы.
3. В случае сохраняющейся ирритативной симптоматики, отсутствии воспалительных процессов нижних мочевых путей и неэффективности альфа-адреноблокаторов после перенесенной РПЭ у ряда пациентов целесообразно назначение М-холиноблокаторов.
